The coverage duration ranges from 90 days to 12 months. The plan will pay 75% of eligible medical expenses within the United Healthcare PPO network and 60% outside of the network after the deductible is satisfied up to the policy maximum. The plan is not renewable or extendable, and can be cancelled for a full refund before the effective date.
